Group Flight Itinerary (Handbook and Flight Itinerary to be posted March 1)

To finalize your application, mail completed forms below to KIIS by March 20:
1. Program Contract
2. Medical Surrogacy
3. Agreement & Assumption of Risk
4. Medical History
5. WKU Admissions application (non-WKU students only): Undergrad / Graduate

Note: Two of the forms need to be notarized (Medical Surrogacy and
Agreement & Assumption of Risk). You may locate a Notary Public at
your university, public library, local bank, etc.

KIIS Student Orientation (mandatory):
 Saturday, April 9, 2011 at Western Kentucky University in Bowling Green, KY from 1-4 pm CST
